We offer membership tiers within a self-pay model designed to provide flexibility for our patients. If you’re not sure where to start, all packages begin with a 90-minute comprehensive intake visit. During this visit, we’ll create a personalized evaluation and treatment plan, and decide together which package best fits your needs. FOUNDATION

  • $1,225

FAQs

  • All services are self-pay (cash/credit). Superbills can be provided upon request to help you seek reimbursement through your HSA or FSA.

  • Yes, financing is available through PatientFi for eligible care packages.

  • Yes, $525 deposit due at the time of scheduling your initial comprehensive intake visit.   

  • If a patient wishes to extend their membership they can do so at the tiered per-visit price, with associated Tiered benefits: 

    • $350/visit for 3 additional visits at Foundation Tier

    • $325/visit for 6 additional visits at Comprehensive Tier

    • $300/visit for 12 additional visits for Concierge Tier)

  • After the package is complete, patients retain access to as needed follow-up and acute care visits ($375 for 60-min visit) for 1-year.  

    If greater than 1 year lapses from last appointment and a patient wishes to re-establish care we will re-start with a comprehensive intake visit to reassess needs ($425, 60-minutes)

  • What is the cost of Acute illness visit (same/next day when available): $375 for 60 minute visit. Total visit cost including in-office diagnostics, procedures, and medications will not exceed $500 total/visit.

  • Telehealth acute care visit (same/next day when available): $195 for 30 minute visit. 

  • In-home and after-hours visits are considered on a case-by-case basis and may include additional fees.
